Maximo Condition Monitoring

I'm trying to understand Maximo's condition monitoring functionality.  I am using version 7.

Are warnings displayed automatically when a meter reading surpassess the warning limit or does a workflow have to be created?  The same question applies to the action limits.  Does out of the box Maximo trigger work orders as an action or does a work flow have to be setup?
